Case study
Jim* is a 26-year-old mechanical fitter.
He was working on a machine when he sustained a crush injury to his right thumb.
Jim’s worksite was in regional Queensland. His injury occurred over a weekend dayshift and he was seen by the on-site nurse who thought that he should be seen by the local doctor.
First, she called the 24/7 RAMMS line.
The Resile doctor was able to talk to Jim and using audio-visual telemedicine could review the symptoms and signs in the thumb.
It was clear that his thumb was swollen and painful around the MCP joint, but normal in all other joints.
He was sent for an X-ray, which was reported by the local radiologist and reviewed by the Resile doctor.
It showed no abnormality at the MCP joint but raised a concern about the CMC joint.
As the Resile doctor had conducted a review by telemedicine, he was able to apply what was found during the assessment to the Xray findings of the CMC joint, and reassure the worker that it was not related to the event.
Jim went home for his off-swing and took it easy on the advice of the Resile doctor.
He was followed up 2 days later and he had fully recovered.
The Resile doctor called him again on the morning of his first swing back, but there were no concerns and he was able to return to work as planned.
