Health Management Plans

Introduction

We believe that a healthy workforce is the cornerstone of a successful business. A Health Management Plan is a program guided by an occupational doctor or Allied Health professional that is designed to assist in enabling your employee to take control of their health conditions that may impact workability.

Why do you need it?

Many health problems contribute to human factor risk, for example, elevated BMI is a risk factor for issues working in heat, musculoskeletal disorders and sudden incapacity, uncontrolled diabetes is a risk factor for sudden incapacity.

 

Health management plans assist in managing individual health risks or chronic diseases to keep employees safe for work.

How does it work?

The trigger for a Health Management Plan is typically identified from a recent medical assessment such as a pre-employment medical, periodic medical or can be recommended from other Cases services such as a Fit for Work assessment.

 

Our current programs can assist with:

  • diet and weight management
  • long-term health conditions such as diabetes, cardiovascular disease and hypertension
  • pregnancy care.

 

These plans:

  • typically take place over the course of a year, however can be tailored to employer and employee needs
  • manage chronic conditions to ensure ongoing fitness for work (or return to a state of fitness for work), aid in preventing complications and improving quality of life
  • provide evidence-based health advice, including nutritional guidance and exercise advice as indicated
  • provide tailored lifestyle coaching
  • provide specialised support to ensure healthy pregnancy while remaining safely in the workforce
  • enable the employee to openly discuss barriers to their progress over the course of the year and provide strategies for overcoming those.

 

Case Example

An operator was referred for a Health Management Plan post routine pre-employment medical, after a high weight and BMI were recorded. 

 

The employee was enrolled in a 12-month plan, receiving four teleconsultations with an allied health professional specialising in weight management, physical activity and nutrition.

 

Each consultation was tailored to the aspects of lifestyle modification that would best support the employees specified goals. In addition to the consultations, the worker was provided with resources assessing their eating habits, provide advice on nutrition and portion control guidance.

 

At each teleconsultation the worker provided their weight to ensure accountability. A report was provided to the employer at the conclusion of each session outlining progress, goals, and discussion points.

 

Over the course of the program, the worker demonstrated a commitment to the plan resulting in the substantial weight loss of 26kgs, significantly reducing their BMI by 26%.
